Things you need to know:

Please contact the Health Benefits Department for the forms that you need to use these services.

The member must sign the form to authorize services.

Advise your doctor that payment takes 4-6 weeks should you choose to use someone not on this list.

If asked to pay for services up front, submit the form with a COPY of your receipt and proof of a vision exam for reimbursement according to your plan.

NOTE: Before using this list please contact the Health Benefits Department regarding eligibility and the plan allowance you have available.
