Things you need to know:

Orthodontic Care is available to Food members and Platinum Plus Drug Members.

Please contact the Health Benefits Department for the forms that you need to use these services.

The member must sign the form to authorize services.

Advise your orthodontist that pre-authorization is required for any Orthodontic services rendered.

NOTE: Before using this list please contact the Health Benefits Department regarding eligibility and the plan you have available.
