Disability Vacation

If you are on State Disability or vacation it is important you contact the Health Benefits Department regarding your coverage.

If you are already familiar with the process you may request disability forms HERE.  

Food Division members may request Vacation Waivers HERE.

Drug Division members will receive vacation questionnaires by mail through the Fund Office automatically.

Be sure to also check with the Membership Dues Department regarding withdrawal status should you be gone for an extended period of time.
